GREENVILLE -- Jacqueline "Jackie" Perkins Forehand, 60, died Tuesday afternoon at University Hospice in Greenville surrounded by her family.
Funeral services will be held Saturday at 6 p.m. at Unity Free Will Baptist Church in Greenville, with the Rev. Jeff Manning officiating. Interment will be held Sunday at 3 p.m. at Wayne Memorial Park in Goldsboro.
Jackie was born in Indianapolis, Ind., June 4, 1948, to Vaughn and Thelma Guion Perkins of Indianapolis.
She had worked as a transcriptionist for East Carolina University School of Medicine.
Jackie was a member of Unity Free Will Baptist Church in Greenville.
In addition to her parents, Jackie is survived by her husband, Charles Lester "Chuck" Forehand; her sons, David Richard Forehand and wife, DeLyn, of Greenville and Johnathan Charles Forehand of Manchester, N.H.; her brother, David Robert Perkins of California; and her grandson, Jackson Chase Forehand.
Flowers are welcome or memorials may be made to the American Cancer Society, 930B Wellness Drive, Greenville, N.C., 27834.
The family will receive friends following the service at the church.
