Jerry Maurice Adams Sr., 34, died Saturday, Nov. 15 at the University of Alabama Birmingham Hospital.
The funeral will be at 1 p.m. Saturday at Woods Chapel Free Will Baptist Church.
The burial will be at Wayne Memorial Park.
The visitation will be from 6 to 8 p.m. Friday at Howell Funeral Home and Crematory.
He was born Dec. 5, 1979, in Flushing, N.Y.
He is survived by his parents, Dennis and Louis Adams, of Dudley; his wife, Tech. Sgt. Natika Horton Adams; and two children.
