Duplin school sign-ups on April 16

Pre-kindergarten/kindergarten registration for Duplin County Schools will be held on April 16. Registration will be at all elementary schools from 8:30 a.m. to 3:30 p.m. Students must be five years old on or before August 31 to enroll. In order to complete an application for pre-kindergarten, students must be four years old on or before August 31. Please bring your child to be registered.
Duplin County Schools require students to have the following:
*A certified birth certificate.
*Immunizations: Diphtheria, Tetanus, Whooping Cough, Polio, Red Measles (Rubeola), German Measles (Rubella), Mumps, HIB (if applicable), Hepatitis B and Varicella. Must bring a record of immunizations that have been administered.
*A health assessment is required by law that has been completed within the 12 months prior to the beginning of school. The health assessment may be obtained at your physician's office, a clinic or your local health department. The health assessment must be documented on the blue state health form.
*911 address and proof of residence (i.e., utility bill, phone bill, water bill, rental contract/lease, etc.) are also needed for school records.
Prekindergarten/kindergarten students who are currently enrolled will not attend school on April 16.
