GLOUCESTER -- Dr. David John "Jack" Middleton Jr., 79, died Wednesday at Sunny Brook Rehabilitation Center in Raleigh.
Funeral services will be held Saturday at 11 a.m. at Warsaw Methodist Church in Warsaw. Visitation will begin at 10 a.m. at Warsaw Methodist Church. Following the service, burial will be in the Devotional Gardens.
Dr. Middleton was born in Norfolk, Va. to the late David John and Carrie Chadwick Middleton.
Dr. Middleton is survived by a son; a daughter; and five granddaughters.
Arrangements are being handled by Community Funeral Home of Warsaw Inc.
